One-story, 45,312 square foot Community-Based Outpatient Clinic with 250 surface parking spaces to be built on approximately 9.5 acres located .5 miles from I-70 and IN Route 46 on Bill Farr Drive. This facility will feature primary care service (PACT), mental health, pathology /laboratory, eye clinic, audiology, imaging, including a mobile imaging pad, prosthetic, and physical rehabilitation.
